ELIGIBILITY

Employees

All full-time employees are eligible to participate in our Medical insurance plans. Full-time employees are eligible to enroll in our health and welfare benefit plans the first of the month following their start date. Per Diem employees may be eligible to elect Medical coverage providing they work an average of 25 hours or more weekly, during the annual measurement period.

All employees are eligible to participate in the employer-sponsored 401(k) Plan the first of the month following 30 days of employment.

Dependents

You may also elect coverage for your dependents in some circumstances. Eligible dependents may include the following: your legal spouse or your same sex domestic partner and your dependent children. Each insurance company, as well as state law, dictates the rules for dependent children under each plan. For example, under our medical plan, you can cover your children up to age 26 regardless of their financial dependence on you and in some cases, you may cover them up to age 30.

The term dependent child includes:

  • A natural or legally adopted child
  • A foster child, if placed in your home with state statues prior to their 18th birthday
  • A spouse or same sex domestic partner’s child residing with you and dependent upon your support
  • A child whom you or your spouse / same sex domestic partner have a legal obligation to support, even though not living with you
