At Infinity, we understand different types of patients can have very different needs. That’s why we’ve created specialty care programs that keep your patient’s specific home care requirements in mind. 

 

Medication Managemant

…..a better way of managing medications for the elderly in their home. Raising the standard of medication practice, Infinity’s Medication Management Program reduces the risk of medication errors and improves compliance by addressing 5 broad medication problems:

  • Unnecessary duplication of medications obtained from various physicians, and/or duplication of the same generic and brand name medication.
  • Cardiovascular medication problems resulting in orthostatic side effects.
  • Use of psychotropic drugs in patients with possible adverse psychomotor effects.
  • Use of non-steriodal anti-inflammatory drugs (NSAIDs) in patients at high risk of peptic ulcer complications.
  • Use of Warfarin and interacting drugs with in-home anticoagulant monitoring.

 

Congestive Heart Failure

Infinity HomeCare offers the Congestive Heart Failure Program to patients with newly diagnosed or exacerbated CHF. Dedicated to improving quality of life for these patients, the goal of the program is to improve functional abilities through symptom and lifestyle management. We provide all the tools for the patient to be compliant and gain independence through self-management skills.

 

Cardiac Care

Types of Patients: Patients with newly diagnosed or exacerbated illnesses such as:

  • CAD
  • CHF
  • Acute MI
  • Angina
  • HTN
  • Arrhythmia
  • Emboli-thrombusPatients Post– Surgery: CABG / Angioplasty / Pacemaker Insertion

 

Day Surgery

With an increase in surgical procedures now being performed on an outpatient basis, elderly patients have a need for medical follow-up once they return home. These patients may live alone, or have an elderly spouse with their own medical problems or limitations, and may require some assistance for a short time after surgery. That’s why Infinity has created our Day Surgery Program.

 

Neuro Rehab

Infinity HomeCare’s Neuro Rehab Program is perfect for patients with newly diagnosed CVA, TIA, Parkinson’s disease, subdural hematoma, brain tumor, or vestibular dysfunction. This program is also appropriate for patients recovering from spinal surgery, brain surgery, head trauma, patients with decline in functional status, and patients with neurological deficits such as:

  • Gait disturbance
  • Balance disorder
  • History of falls
  • Ataxia
  • Vertigo
  • Pain
  • Weakness
  • Hemiplegia
  • Apraxia
  • Dysphagia & Dysphasia

 

Pulmonary Care

Our Pulmonary Care Program  is designed for patients with newly diagnosed or exacerbated illnesses such as:

  • Pneumonia
  • Acute Bronchitis
  • COPD
  • Asthma
  • Emphysema or Fibrosis
  • Patients recovering from Pulmonary Surgery
  • Patients requiring Respiratory Treatments and / or Oxygen Therapy
  • Patients with Pulmonary Neoplasms and related diseases

 

Welcome Home

This program is specifically designed for patients returning home from an extended stay in a hospital or facility. We will provide same day admission to home health care services. A registered nurse will visit the patient the same day they return home from your facility, making the transition less stressful.
